We identified eight areas of organisational learning during a Death or Serious injury (DSI) investigation.

Recommendations
The IOPC recommends that Derbyshire Police standardise terminology within the local policing unit (LPU) and the control room for the lists of jobs to be managed and dealt with to avoid any confusion as to who is responsible for what.
The supervisors within both the LPU and the control room, and the officers within the LPU refer to the lists by different titles which caused confusion and led to the North Town (NT) list not being monitored or managed.

Accepted action:
As a result of the Gold Group recommendations, a meeting was held with both North and South Command Teams and a user guide was developed and has been shared with all uniformed supervisors. At the time of writing, the lists appear to be being monitored although this will remain under review hence the Amber grading.
The IOPC recommends that Derbyshire Police introduce a policy and guidance for the management of the different lists of jobs, so supervisors are aware of their responsibilities in relation to them.
The supervisors within the LPU were not aware of all of the different lists they were responsible for and therefore outstanding tasks were not viewed.

The IOPC recommends that Derbyshire Police provide the same training to Local Policing Unit (LPU) and control room staff in relation to ControlWorks system and in relation to vulnerability, risks, safeguarding and THRIVE.
Staff within the LPU explained they had very little training in these areas and therefore were not fully equipped to complete their role and this has been confirmed by the OLCE department.

Accepted action:
The force has recently (February 2020) commissioned full THRIVE and Vulnerability training for all uniformed staff which will roll out over the next 6 months. This has been graded as Red until the training has been completed. We do not however have the responsibility for Control Works training.
This training is current on hold pending the Covid19 outbreak and as a result no training is occurring. The 6 month time limit will undoubtedly slip.
Red grading
The IOPC recommends that Derbyshire Police implement a policy to ensure a clean handover from one LPU shift to the next takes place.
No handovers were recorded at shift change times. The IOPC is aware that Derbyshire have implemented some fast time learning around this issue. However, the IOPC request that the force updates us as to whether this has been successful.

Accepted action:
This task was allocated to two sergeants from each Division. Since that time one of them has been re-deployed because of the Covid19 virus and the other is currently off sick having developed appendicitis.
This was the last update:
‘…We had identified some common ground around the handover document and standardising the approach of LPU Sgts updating CMARC, but not much further I’m afraid.’
This will be picked back up once this medical emergency has been resolved.
Red grading
The IOPC recommends that Derbyshire Police clarify and/or implement/formalise an escalation process when there are outstanding matters to be resourced.
Due to the lack of policy, some staff members inform their supervisors whilst other do not. This incident was left outstanding for 26 hours. The IOPC is aware that Derbyshire have implemented some fast track learning around this issue. The IOPC request that the force updates us as to whether this has been successful.

Red Grading
The IOPC recommends that Derbyshire Police implement a supervision policy around the closure of incident logs.
In this particular case, two staff members were not clear as to who could close an incident where the original classification code was different to the closing classification code.

Accepted action:
As a result of Op Marlow, a more robust audit process is now in place. One of the auditors completes monthly checks of the EO (Enquiry Office) staff with regards to opening and closing codes. She has also provided additional training to one of the EO Managers who also QA's the work of the EO Staff. Any new training that is provided to Force Operations Room staff is also provided to the EO staff.
Amber Grading due to ongoing QA of opening and closing codes (although no enquiry office are open at the moment due to Covid19)
The IOPC recommends that Derbyshire Police review their training around THRIVE in relation to re-THRIVE and re-grading an incident when having contact with an individual.
The wording on this slide is contradictory and confusing as it states that the incident should be re-THRIVED if they are unable to make contact with an individual and then further down states contact is required with an individual.

Green Grading
The IOPC recommends that Derbyshire Police implement a policy to ensure that there is consistency in how LPU sergeants inform the control room staff about availability of their staff to be deployed.
Some sergeants provide the control room with a list of available officer for deployment whereas others do not and the control room have to contact them and ask.
