Investigation into police contact with man prior to his death - West Mercia Police, October 2022

In October 2022, West Mercia Police received a report of an assault. During the course of several calls made to the police, information was passed to them regarding a vulnerable adult male and a concern for his safety was raised. 

On the 23 October the male was found dead at his home. 

During the course of the investigation, body worn video footage was reviewed, radio communications were obtained and audio recordings of the calls were analysed. Police logs were reviewed and accounts were taken from police officers and police staff.

The investigation concluded there was no indication any police officer or member of police staff may have behaved in a manner that would justify the bringing of disciplinary proceedings or committed a criminal offence.

We carefully considered whether there were any learning opportunities arising from the investigation. We make learning recommendations to improve policing and public confidence in the police complaints system, and prevent a recurrence of similar incidents.

In this case, the investigation has not identified any organisational learning.
